Our Unwavering Standard of Care Across All Locations:

No matter the location, Instacare Wound Care adheres to a rigorous and patient-centered care model, ensuring excellence in every interaction. Our comprehensive approach includes:

  • Round 2-3x per week.​

  • Utilization of non-sharp debridement.​

  • Rounds M-F, during routine business hours, while SNF Wound Team is present.​

  • Communication during grand rounds with wound team, patients, families, and leadership in facility.

  •  At home care timely documentation.​

  • Lead five principles of wound management: ​

                   1) wound assessment, 2) wound cleansing, 3) timely dressing change,

                  4) selection of appropriate dressings, 5) antibiotic use​

  • Note documentation in the facilities and completed 24-48 hours.​

  • Treatments are assessed and new treatments are approved and changed to insure reduced healing time.

  • Stay within your formulary and followed for greatest cost impact without risking quality healing wounds.

  • Assure appropriate antibiotic medications, offloading DME, and specialty provider referral/orders are expedited.

  • Leadership, patient’s primary care, and families are provided the most up-to-date information on patients  current wound status, changes and treatments.

  • Attend QAPI, Care Management etc. facility meetings to provide wound care education and patient outcomes updates.

  • Ongoing clinical support with providers ¢ Education/ In-Servicing.
